We understand the importance of living in the community – it is therefore our goal to assist in being able to do so.
Our “Nursing Home Transition and Diversion Program” is designed to empower individuals to be able to live independently in the community. This program provides an alternative to institutionalization for both people with disabilities and seniors.
Regency is approved to provide the following Services:
Home and Community Support Services (HCSS)
HCSS provides a participant with a Personal Care Aide who can assist with oversight and/or supervision to safety monitor an individual from harmful activities (for example wandering or leaving the stove on unattended). Additionally, they can assist with Activities of Daily Living (ADL) such as dressing, bathing, hygiene/grooming, toileting, ambulation/mobility, transferring and eating, and/or Instrumental Activities of Daily Living (IADL) such as housekeeping, shopping, meal preparation, laundry, transportation.
Every participant in NHTD is provided a Service Coordinator. It is the job of the Service Coordinator to assist each participant in obtaining and coordinating the services that are necessary to return to or remain in the community. Additionally, the Service Coordinator will assist with the development and implementation of a person-centered individualized Service Plan. This plan will support the participant to obtain independence and integration into the community, as well as assure his or her health and welfare. The Service Coordinator also works with the participant to find and secure housing.
