HHA’s work under the guidance of an qualified RN and are given a Plan of Care customized to your needs.
As opposed to CDPAP, when receiving HHA services it is our responsibility to properly train and schedule your HHA in addition to managing your care.
Schedule an assessment with the Conflict-Free Evaluation and Enrollment Center (CFEEC) to ensure you meet the requirements to receive Managed Long-Term Care (MLTC). Enroll into a Managed Long-Term Care Plan of your choice (MLTCP).
Have a RN from the MLTCP perform an assessment to create a personalized care plan for your needs. This includes the amount of home care hours you will be authorized for.
Once you are enrolled into a MLTCP and receive your care plan you can begin receiving HHA services on the 1st day of the following month.
